Form F in short steps

PCPNDT register and monthly report
Every signed Form F goes into your register in serial order. The monthly report for the Appropriate Authority downloads as Excel, and each Form F prints as a PDF.
Reports on your letterhead

Patients and referrals
Find anyone by phone number or name. Each Form F records who referred the patient.
Bills and receipts
Add the charge for each scan to the patient's bill and give a receipt for every payment.
Form F record safeguards
Form F records cannot be deleted through the workflow, corrections are added and tracked, and the form has no field for recording fetal sex. These functions do not provide legal advice or government approval.

Do I need my PCPNDT registration to sign up?
Not to sign up. You add your centre's PCPNDT registration number in Form F, and forms can't be signed until it's there.
Can two sonologists work at one centre?
Yes. Your plan includes two sonologists and three staff logins. Need more sonologists? Talk to us.
Does Everchart record the sex of the fetus?
No. There is no field for it anywhere, by design.
Is there a free trial?
No. You see your price when you sign up and pay once for the year. Your year starts the day we approve your centre, and we call you on the number you give us to help you start.
How do I pay?
Once a year, online by card, UPI or net banking.
